MultiMC is portable: instances live in an instances folder next to the launcher itself, not in AppData. Each instance carries its own .minecraft with saves, mods, configs and screenshots fully isolated.
Worlds sit in the instance's .minecraft\saves tree exactly like vanilla. Copy one instance folder and the modpack plus worlds moves as a unit.
Where MultiMC stores this, by platform
<MultiMC>\instances\<Instance>\.minecraft\saves
Replace <MultiMC> with wherever you extracted the launcher and <Instance> with the instance name. Worlds are per-world folders with level.dat inside. Mods, configs and screenshots sit in sibling folders of the same .minecraft. Use Folders > View Instance Folder in the app to jump there directly.
