Baldur's Gate 3 mods are single .pak files dropped into the Mods folder next to your saves. Load order lives in modsettings.lsx inside PlayerProfiles\Public, which mod managers rewrite every time you export. Hand editing works but one malformed entry resets the file.

Yes. Patch 9 moved saves, profiles and mods together from Documents to AppData Local. The game copies old profiles across on first launch, which can also drag stale mods along. Clean the Mods folder after the move.

How do I back up my mod list?

Copy the Mods folder plus modsettings.lsx from PlayerProfiles\Public. Saves live in the Story folder beside it, so grab that too for a full backup. Never nest mods in subfolders; the game resets modsettings.lsx when it finds them.
