Where Does Diablo IV Store Settings?

Diablo IV characters live on Blizzard servers, so there is no save file to back up. What sits on your disk is LocalPrefs.txt with graphics, sound and intro skip preferences inside Documents/Diablo IV. Losing it only resets options.

When the game forgets settings or replays the opening cinematic every launch, that file is usually corrupt or cloud synced badly. Rename it with the game closed and Diablo IV builds a fresh one. Exclude the folder from OneDrive if the problem returns. Console versions keep settings in cloud storage with no local file at all.
